How do I reduce no-shows at my clinic?

Send reminders people can act on, make rescheduling easy, explain your policy clearly at booking and follow up quickly when someone misses. A fee can help in some clinics, but it should be fair, written down and consistent. Start by finding out why people miss.

  • Numbers you can check

Our figures come from a public scan of clinic homepages, and we publish how we counted.

Understand your pattern

Look at your booking software’s records. Which appointment types are missed more than others? New or returning patients? Which days and times? Early morning, late afternoon and first visits often behave differently. A pattern tells you which fix to try.

Common reasons include forgetting, a change in plans, difficulty getting time off, transport problems and a patient feeling better. Some of those are easy to prevent. Others you can only shorten the damage from.

Reminders that work

  1. Confirm at booking. A message straight after booking that includes the date, address and parking.
  2. Remind ahead. Send at least one reminder a day or two before, and perhaps a second closer to the time.
  3. Make it one tap. Include a link to confirm, reschedule or cancel.
  4. Use the channel patients prefer. Text or email, as your software allows and your consent records permit.
  5. Say what to do. “Reply or tap here if you can’t make it, so we can offer your time to someone else.”

Booking tools such as Jane and Cliniko offer automatic reminders. Check your settings and turn on what your plan offers. Messages are subject to consent rules on commercial and service messages, which vary by country and province or state. See email consent.

Make rescheduling easy

A missed appointment is often a patient who couldn’t face a phone call. Let them change the time online, and keep a waitlist so a freed slot can be filled. A quick response to a late cancellation can rescue the slot.

A clear policy

Put your cancellation and no-show policy in writing and show it at booking, in confirmation messages and at the front desk. Say how much notice is needed and what happens if it isn’t given. Be consistent, because exceptions invite arguments.

Fees are a business decision. Some clinics charge, some waive the first miss and some use a deposit for certain bookings. Rules about fees and consumer protection differ by province and state, and your regulator may have views. This is not legal advice. For more on policies, see no-show policy ideas.

Not sure where your clinic stands? We’ll look at it the way a new patient does and send a short written report. No pitch.

Follow up kindly

When someone misses, contact them the same day, without blame. Offer a new time and ask if anything made it hard. Over time you will see the real reasons and be able to fix the cause.

Where marketing fits

No-shows waste the work that went into attracting the patient. If first visits are missed more than later ones, look at the booking path and the information given. A patient who knows what to expect, has a reminder and booked an appropriate slot is more likely to arrive. See reduce patient no-shows.

Questions owners ask

Should I charge a no-show fee?

It depends on your clinic, patients and local rules. If you do, state it clearly at booking and apply it consistently.

What’s a good no-show rate?

There is no single figure. Track your own rate by appointment type and aim to reduce it.

Do deposits work?

They can for some bookings, but they add friction at booking. Test on a small scale first.
