How much do clinic marketing agencies cost?

The honest answer is that it depends on the model and the scope, so any single number you read online is a guess. Agencies that serve clinics usually bill in one of four ways: a monthly retainer, a one-off project, a low-cost template plan, or a fee per lead. Ad spend is almost always separate and paid to Google or Meta directly.

The four ways agencies charge

Monthly retainer. You pay a set amount each month for a defined list of work: website updates, online profile care, reporting, perhaps ads management. It suits clinics that want steady attention. The risk is paying for activity rather than outcomes, so ask for the list of tasks in writing.

One-off project, then a smaller monthly fee. A new site, a booking set-up or a clean-up of your online profile is quoted as a fixed job. After that, a lighter monthly plan covers hosting, small edits and reporting. This is easy to budget because you can see where the big spend ends.

Template plans. Some providers sell a low monthly price for a site built from a shared template. It can be a fair start for a new solo practice. The trade-offs are limited design choices and, sometimes, a site that is hard to move if you leave. Check who holds the domain and the files.

Pay per lead or per booking. It sounds safe, but ask how a lead is defined. A form fill, a call of any length and a booked first visit are very different things, and a fee per lead can push an agency toward volume over fit.

What moves the price

Four things move the price. The first is the number of locations, because each one needs its own online profile, page and tracking. The second is the scope: a website refresh is a smaller job than a rebuild plus ads plus email. The third is how much content you want written, such as condition pages and posts. The fourth is how much you want to hand over versus keep in house.

Ad spend is a separate line. If you run Google Ads, you pay Google from your own ad account, and any management fee is on top. Be wary of any quote that bundles ad spend and fees into one number you cannot see inside.

What to ask before you compare quotes

Put every quote into the same shape so you can compare it:

  1. What exactly is included each month, listed as tasks?
  2. What is the setup cost, and is it one-off or recurring?
  3. Is there a minimum term, and what does it cost to leave?
  4. Who owns the website, domain and ad accounts?
  5. What will the monthly report show, and can you see the numbers yourself?

A cheaper quote that excludes reporting or ownership can cost more later than a higher one that includes them.

How to judge whether the cost is worth it

Work out what a new patient is worth to your clinic across a typical course of care. Then count the booked first visits that came from marketing. If you cannot count them yet, fix tracking first. See how to measure booked first visits.

Ours are custom packages, scoped in writing after an audit and described on our pricing page. You choose whether to pay monthly or as one project.

Not sure where your clinic stands? We’ll look at it the way a new patient does and send a short written report. No pitch.

Questions owners ask

Is the cheapest agency a bad sign?

Not by itself. Low prices can reflect a narrow, templated job. Check what is left out, such as reporting, edits and ownership of your accounts.

Should I pay an agency a percentage of my ad spend?

It is a common arrangement, but it means the agency earns more when you spend more. Ask for a flat fee, or at least a cap, and review it against results.

Can I pay for one piece of work instead of a monthly plan?

Often, yes. Many projects, such as a booking set-up or profile clean-up, can be quoted as a fixed scope with no ongoing commitment.
